Hospital to Home Leadership

Please be sure to view the video stream link prior to attending your session. Hospital to Home (H2H) is a student-led clinical experience aimed at connecting hospitalized patients at high risk for unmet social and medical needs with the appropriate resources. The program aims to improve health outcomes and reduce the need for future hospitalization. Students in the program help facilitate a successful transition of care by providing basic medical equipment, assessing social determinants of health, and identifying community resources to bridge those gaps. You need access to St. Vincent’s Epic Context while volunteering with H2H, please make sure to activate your access prior to volunteering.
a. If you have never volunteered at St. Vincent’s and don’t have access then you will need to go to this website: https://bit.ly/3JZkmoL (need to be on UTMB network to access), St. Vincent's Clinics (utmb.edu) and complete the training: –> Current MS2-3-4/…. The first one on the list.
b. If you have logged-in to St. Vincent’s Epic before but it has been a while (longer than 90 days), you will need to call the help desk to have them reactivate your access – 409-772-5200.

Spanish Speaker Assistance at St. Vincent's Clinic

**If you register for this session, please click the link in the registration field below to access orientation materials, timesheet and questionnaire**
Our goal is to support Spanish-speaking patients in activities surrounding their St. Vincent’s Clinic appointment. We want to ensure that patients can navigate clinic information surrounding their visit with confidence by providing guidance in their own language.
Main tasks students in the front desk can help with:
    1. Make phone calls to confirm attendance of patients to the clinic
    2. Make phone calls to provide patients with test results. (This is a really important need in the clinic, since we were told that sometimes notification of results to Spanish speaking patients gets pushed back until someone who can actually do it shows up to the clinic).
    3. Fill out the “check in” questionnaire for patients arriving for their appointment
    4. Answer any questions the patients may have during their visit
    5. Have someone that guides patients to their room and guides them in the process of scheduling follow ups and any other consultation in other clinics.
