This clinic cares for recently hospitalized patients with comprehensive care including with medications, nursing, and social work assistance. This clinic helps patients successfully transisition out of the hospital and transition care to a new medical home.

To deliver groceries & hygiene supplies to over 140 families in crisis that have no mode of transportation on Tuesdays, Wednesdays, and Thursdays in personal vehicles & alongside with other volunteers. Dress weather appropriate with active wear & closed toed shoes. Must be able to lift up to 25 lbs. use personal vehicles & work with other volunteer.
A clinic providing integrated, interprofessional care to medically, socially complex uninsured patients. The health professions providing care at the interprofessional clinic include clinical lab sciences, licensed counselors, medical doctors, nursing, occupational therapy, pharmacy, respiratory care, and social work. This clinic is staffed by students that are part of the I-COPE program, students that have committed to providing longitudinal care to the underserved over the course of their third year of medical school.
