Hospital to Home (H2H) is a student-led clinical experience aimed at connecting hospitalized patients at high risk for unmet social and medical needs with the appropriate resources. The program aims to improve health outcomes and reduce the need for future hospitalization. Students in the program help facilitate a successful transition of care by providing basic medical equipment, assessing social determinants of health, and identifying community resources to bridge those gaps.
