Hospital to Home Leadership

Please be sure to view the video stream link prior to attending your session. Hospital to Home (H2H) is a student-led clinical experience aimed at connecting hospitalized patients at high risk for unmet social and medical needs​ with the appropriate resources. The program aims to improve health outcomes and reduce the need for future hospitalization. Students in the program help facilitate a successful transition of care by providing basic medical equipment, assessing social determinants of health, and identifying community resources to bridge those gaps. You need access to St. Vincent’s Epic Context while volunteering with H2H, please make sure to activate your access prior to volunteering.
a. If you have never volunteered at St. Vincent’s and don’t have access then you will need to go to this website: https://bit.ly/3JZkmoL (need to be on UTMB network to access), St. Vincent's Clinics (utmb.edu) and complete the training: –> Current MS2-3-4/…. The first one on the list.
