Hospital to Home (H2H) is a student-led clinical experience aimed at connecting hospitalized patients at high risk for unmet social and medical needs​ with the appropriate resources. The program aims to improve health outcomes and reduce the need for future hospitalization. Students in the program help facilitate a successful transition of care by providing basic medical equipment, assessing social determinants of health, and identifying community resources to bridge those gaps. Please confirm you can access St. Vincent’s Epic Context ASAP.

During this outpatient dermatology shadowing experience, you will observe how dermatologists evaluate and care for patients with a variety of skin conditions. You will gain early exposure to the dermatologic history and physical examination, including how skin findings are described and interpreted, while observing patient-centered communication, clinical reasoning, and the flow of care in an outpatient specialty clinic. This experience is designed to help you connect foundational science and clinical skills with real-world patient care.
Dress Code:
- Scrubs suitable for clinic use and white coat.
- You must have your student ID visible. You will not be allowed entry without your student id.

Psychiatry Clinical Experience

Review recorded patient interview, write H&P with differential diagnosis and initial plan for further evaluation/treatment, and prepare for oral presentation prior to session. During session, students will give an oral presentation to the facilitator in front of peers. Facilitator will provide feedback to oral presentation and written H&P. Will discuss differential diagnosis and management plan as a group. Dress Code: Business casual/usual clinic attire preferred
