Hospital to Home (H2H) is a student-led clinical experience aimed at connecting hospitalized patients at high risk for unmet social and medical needs with the appropriate resources. The program aims to improve health outcomes and reduce the need for future hospitalization. Students in the program help facilitate a successful transition of care by providing basic medical equipment, assessing social determinants of health, and identifying community resources to bridge those gaps. Please confirm you can access St. Vincent’s Epic Context ASAP, even before your first day, as it may take a couple days for activation. • ALL MS1s and most MS4s will already have access to St. Vincent’s Epic so you will NOT have to go the link below.
